Reporting your child’s immunizations to public health

Every time a child is immunized by a health care provider, families must report it to their local public health unit. Please do not report immunizations to your school. Ontario school boards do not keep student immunization records.

Haldimand Norfolk Health Unit Immunization Reporting (covering Haldimand County and Norfolk County)
